Brooke Kuhlmann 8528e0ff01 Removed CHANGELOG.md (use CHANGES.md instead).

- Simplifies the name since "Log" is implied and is one of the three
  common file names for this type of information: CHANGELOG.md,
  CHANGES.md, and HISTORY.md
